Kozak and his wife Janet, loving grandmother of Chelsea Sigismondi, Alyssa, Derek and Madelyn Kozak. Best friend of long-term healthcare provider Jerome S. Kozak, best friend Betty Lou Kelly, estranged wife of Gregory G. Kozak Sr, and also survived by many loving family and friends.
