A 34-year-old man whose last known address was in Montgomery County was charged last night with first-degree murder in the fatal shooting of a prominent suburban Chicago plastic surgeon at his office Friday, Illinois authorities said.

Jonathan Preston Haynes, whose last permanent address was in Rockville, was charged by the Cook County state's attorney with killing Dr. Martin R. Sullivan, 68, who was shot at least four times in the chest by a small-caliber handgun.

Mr. Haynes, who was arrested late Saturday, was ordered held without bond, pending a hearing today at the Skokie Courthouse.

Mr. Haynes was arrested after a Skokie, Ill., police officer stopped a 1968 powder-blue Volkswagen Beetle with Maryland license plate number ZSS 499 -- the same bumper-less vehicle described by witnesses as leaving the vicinity of the crime Friday afternoon. That car, according to Maryland Motor Vehicle Administration records, is registered to Mr. Haynes, said Wilmette Police Chief George E. Carpenter.

Neighbors said yesterday that he had moved from his home on Ivy League Lane about a month ago.

Dr. Sullivan, a North Shore plastic surgeon and head of cosmetic surgery at Evanston Hospital, was shot by someone who had made a 2:45 p.m. appointment Friday, using the name John Rothman.

The suspect -- described by witnesses as a thin, balding white man wearing glasses and a blue suit -- was shown into the examining room, where he was joined by Dr. Sullivan minutes later.

The staff then heard gunshots and saw the man run from the office.
